New Client Paperwork
To help us make the most of your first appointment, please complete your intake paperwork prior to your session. You can complete all forms in your client portal. During your initial session, your counselor will also review your rights and responsibilities as a client. Courtney is committed to protecting your privacy, and you can expect your personal information to remain strictly confidential in accordance with state law.
Therapeutic Services for Minors
To ensure your child gets the best possible support, the first session is a parent consultation. Courtney will meet with you, and your spouse/partner, if available, to discuss your goals, share insights, and create a care plan tailored to your child’s needs. If your child is subject to a divorce decree or any legal court order regarding custody, the state requires a full, complete copy of these documents prior to beginning services. If applicable, please be prepared to provide these documents at the parent consultation, so we can proceed with care.
If you are currently navigating a divorce, please note that while Courtney is fully qualified to provide counseling for your child, she is not a forensic psychologist and does not perform custody evaluations or make legal custody recommendations.
